Overview

Enhanced Care Services provide intensive, integrated support for people whose medical and psychiatric needs are too complex for traditional community settings. Delivered in collaboration with Hood River Post Acute, ECS brings  mental health treatment, and social services together to support the medical services in one coordinated approach so individuals can stabilize, regain skills, and whenever possible return to the least restrictive setting.

Our Process

Referrals and eligibility are managed by ODHS APD and Health Solutions coordinators in Salem. Once approved, MCCFL and Hood River Post Acute work together to ensure a smooth transition into services. Our goal is to make the process clear and supportive from the very beginning.

Intake and Assessment

We begin with a comprehensive evaluation of medical, psychiatric, and behavioral needs. This helps us understand your unique situation and set personalized goals.

Interdisciplinary Care Planning

Our team collaborates to create a unified plan that includes therapeutic interventions, medication management, and community integration activities tailored to your needs.

Ongoing Support and Transition Planning

We provide consistent care and review progress weekly. From day one, we plan for the next setting, coordinating with families, providers, and community resources to support a successful transition.

Enhanced Care Outreach Services

Enhanced Care Outreach Services are designed to support individuals with complex behavioral health needs who live in community settings or are transitioning from higher levels of care. This outpatient service provides proactive engagement, individualized planning, and coordination with caregivers and providers to promote stability and safety. Outreach teams focus on reducing hospitalization risk, addressing behavioral challenges, and enhancing quality of life through skill-building and crisis prevention, ensuring individuals can thrive in the least restrictive environment possible.
