Before your first appointment at Mid-Columbia Center for Living, please complete the intake paperwork. You can do this online below, request a printed copy, complete it over the phone, or in person. This helps us understand your needs and ensures a smooth start to services. Our Referral Coordinators are available if you need assistance.

Additional documents including detailed policies can be found on our Forms Page.

Important Notes

  • Intake paperwork must be completed prior to your first appointment.
  • For children under 14, a parent or legal guardian must provide consent for care and will be asked to participate in the first appointment or be available for consultation.
  • You may also complete Release of Information forms for important contacts—such as your primary care provider or others involved in your care. These forms help us coordinate services and keep your care team informed.

What Happens After You Complete Your Paperwork

A Referral Coordinator will contact you to:

  • Confirm your information
  • Ask a few basic questions about you to help set up your services and meet state requirements
  • Discuss payment options, including support for uninsured or underinsured clients or those who prefer not to use insurance

What to Bring

  • Your insurance card (if applicable)
  • A list of all medications you are currently taking—both prescription and over-the-counter—including:

    • Name
    • Dose
    • Frequency
  • A list of any medications you are allergic to or have had adverse reactions to in the past
  • Any documents that might be relevant to your treatment, such as:

    • Previous psychological testing
    • Hospital discharge summaries
    • Recent lab results

What to Expect During Your First Appointment

Your first behavioral health visit is focused on understanding your needs and connecting you to the right services. Sometimes, the person you meet with will continue your care; other times, your next appointment will be scheduled with the provider who best meets your needs.

During this appointment, a case manager or clinician will:

  • Review your intake paperwork
  • Ask questions about your health history, current concerns, and goals
  • Complete screenings, which may include written assessments and a urinalysis
  • Explain available services and answer your questions

How Long the Visit Might Take

Plan for 60–90 minutes for your initial appointment.
